The registered well owner must file ADWR’s Pump Installation Completion Report within thirty days after pumping equipment is installed. Obtain equipment details and required well-test measurements from the installer before signing. This owner report is separate from the driller’s well log and does not establish ongoing water quality or future yield.
Is this the owner’s report or the driller’s?
A.R.S. 45-600(B) places the Pump Installation Completion Report on the registered well owner, within thirty days after installation of pumping equipment. ADWR’s current Form 55-56 repeats that responsibility. The agency recommends completing the report with help from the firm that installed the pump.
The driller has a different reporting duty under 45-600(A): maintain the drilling log and file the driller’s report after drilling. A contractor’s well log does not replace the owner’s pumping-equipment report. Arrange who will supply the technical measurements and who will submit the signed owner form before the installation is finished.
Which identification and equipment records are needed?
Use the well’s 55- registration number and the actual well location. Form 55-56 asks for owner and contact information, the location and parcel identifier, equipment installation date, pump type and rated capacity, power type and motor horsepower. It also asks whether a pitless adaptor was installed and its depth where applicable.
Obtain those details from the installer’s records rather than copying a neighbor’s pump specification or guessing from a motor label alone. Keep the proposed equipment choice separate from measured performance: choosing a well pump depends on the well and demand, while this report records what was installed and tested.
What does the well-test section record?
The statute and form require actual test information, including static water level, pumping water level and drawdown, pumping rate, test duration and the methods used for measuring flow and water levels. The statute describes at least four hours of continuous pumping for the specified non-flowing-well capacity and drawdown measurements, with separate provisions for flowing wells.
Have the qualified installer provide measurements collected under the applicable procedure. Do not infer test results from the pump’s rated capacity or conduct an improvised test merely to populate the form. If information is missing or contradictory, resolve it with the installer and ADWR before certifying that the report is accurate.
What should the owner verify before filing?
Use the current agency form and filing instructions, sign the owner certification and retain the completed report with the installation records. The June 2026 form is newer than the agency’s April 2022 instruction sheet; check the current form where contact fields or submission details differ.
The installation-report requirement is not limited to selecting equipment for a newly drilled well. If the work is a repair rather than installation of pumping equipment, ask ADWR whether a report is required for that specific scope. Filing does not establish future well yield or drinking-water safety. Continue the well’s operating and testing responsibilities, and keep drilling or deepening approval separate from this completion record.
